Trying to remove a bookmark that keeps "reappearing"

I have tried deleting the bookmark; sanitizing the document; and saving an optimized version of the document, but nothing works. Every time I reopen the file the bookmark "Tab 01. Note Purchase Agreement" reappears.

Deleting a bookmark should work. Is there any chance that the bookmark is being created by the document when you open it? You can check to see if there is a document level script in the document that will do that.
