Hi

You could try creating a watermark? You can then add the image to the background, control the positioning and the opacity and other option, in Acrobat X onwards, got Tools – Pages – Watermark – Add Watermark. You can then add the file of your choice into the PDF.

Then make sure that you choose 'Appear behind page' in the location section.

The other option of course is to open the image in Photoshop or Fireworks, remove the background of the image and lower the opacity of the image just enough that you see the checkered background. Of course, you want ot create a layer with a white background and change the opacity of the image so it looks right on a white background.

Something to try.
.
--| Using MS Word
Drop the image into a blank Word file.
Use Word's "Set Transparent Color" tool to force the image's background color transparent.
Now create a PDF using Acrobat's PDFMaker.

--| Using Acrobat
Have two PDFs.
PDF_1 is your target PDF.
PDF_2 has the image.
Open PDF_1 then use Acrobat's add Background.
.
Don't know if either will meet your needs.
But, maybe; so, again something to try.
